Experiencing issues with your CPAP device or oxygen concentrator? Please use the forms below to provide information about you and your device. When you’re done, print a PDF of the form, enclose it in a box with your device and ship it to:

For timely service, we advise that patients use UPS or FedEx to ship your equipment. Find your nearest UPS location here or your nearest FedEx location here. If you’re in the Cleveland area, you can also drop your device off between 8:00 a.m. and 4:30 p.m. Monday through Friday. If possible, please complete the appropriate form in advance.

Sending in your CPAP or BiPAP device for repair? The first step is to complete the CPAP Equipment Repair Form to provide details on you and your equipment. Click this link to access the form and instructions about getting your device to us. Please Note: We are experiencing significant parts shortages on ResMed Air10 and S9 CPAP devices. You will be notified when you receive your estimate if the parts needed for your repair are backordered.
