Experiencing issues with your CPAP device? Please use the form below to provide information about you and your unit. When you’re done, print a PDF of this form, enclose it in a box with your device and ship it to:

For timely service, we advise that patients use UPS or FedEx to ship your equipment. Find your nearest UPS location here or your nearest FedEx location here. If you’re in the Cleveland area, you can also drop your device off between 8:00 a.m. and 4:30 p.m. Monday through Friday. If possible, please complete our CPAP Equipment Repair Form below in advance.
